Shoulder (Proximal Humerus) Fracture Treatment in Coachella Valley

A shoulder fracture involving the humerus can be a painful and potentially debilitating injury that affects mobility, strength, and daily function. Most shoulder fractures occur in the upper portion of the humerus, known as the proximal humerus, which forms the ball portion of the shoulder joint. These fractures can occur as a result of falls, sports injuries, or traumatic accidents and are especially common among older adults with osteoporosis. Depending on the severity of the fracture, treatment may range from immobilization and physical therapy to surgical repair. What Is a Proximal Humerus Fracture?

The humerus is the long bone of the upper arm. A proximal humerus fracture occurs when the upper portion of the bone that forms the ball of the shoulder joint breaks. Fractures may be non-displaced (bone remains aligned), minimally displaced, displaced (bone fragments shift out of position), or multi-part fractures involving several fragments. The severity and location of the fracture help determine the most appropriate treatment plan.

Common Causes of Shoulder Fractures

  • Falls — the most common cause, particularly among older adults
  • Contact sports and recreational activities: football, hockey, mountain biking, skiing
  • Motor vehicle accidents causing high-energy trauma
  • Osteoporosis increasing susceptibility to fractures even from minor falls

Symptoms of a Shoulder Fracture

  • Severe shoulder pain
  • Swelling and bruising
  • Inability to move the arm normally
  • Tenderness around the shoulder
  • Visible deformity in severe cases
  • Weakness and pain when attempting movement

Diagnosing a Shoulder Fracture

Your orthopedic specialist may evaluate pain location, swelling, range of motion, neurovascular status, and signs of deformity. X-rays are the primary diagnostic tool used to determine fracture location, bone alignment, number of fracture fragments, and severity of displacement. In complex fractures, a CT scan may provide additional detail to assist with treatment planning.

Non-Surgical Treatment Options

Many proximal humerus fractures can heal successfully without surgery when bone fragments remain aligned and the fracture is stable. Treatment includes sling immobilization to support and protect the shoulder during the initial healing phase, pain management medication, and physical therapy as healing progresses to restore range of motion, flexibility, and strength.

Surgical Treatment Options

Surgery may be recommended when the fracture is significantly displaced, multiple fracture fragments are present, joint alignment is compromised, or conservative treatment is unlikely to provide a good outcome.

Open Reduction and Internal Fixation (ORIF)

Repositions the bone fragments and secures them with plates, screws, or other fixation devices to restore proper alignment and promote healing.

Shoulder Replacement Surgery

In severe fractures, particularly in older patients, partial or total shoulder replacement may be recommended to restore function and reduce pain.

Reverse Shoulder Replacement

Certain complex fractures may be treated with a reverse shoulder replacement to restore function when rotator cuff integrity is compromised.

Recovery and Rehabilitation

Recovery depends on fracture severity, treatment method, patient age, and overall health. Most patients can expect several weeks of immobilization, gradual progression of motion exercises, strengthening rehabilitation, and ongoing monitoring with follow-up imaging. Complete recovery may take several months, although many patients regain excellent function with appropriate treatment.
